You can use the filtering tools on the Find a Doctor or Medical Office page to locate a medical office and click on the address for directions.

  • What should I do if some of the health information in MyACPNY is incorrect?

The health information in MyACPNY is obtained directly from your electronic medical record at your doctor’s office. Your medical record is reviewed and updated during every clinic visit. To correct any inaccurate information, please ask your doctor at your next clinic appointment.

  • Is MyACPNY secure?

To maintain the privacy and security of your health information, MyACPNY requires a secure activation code, personal username, and password to access information. Individual users control their passwords, and the account cannot be accessed without them. Additionally, MyACPNY uses the latest 128-bit SSL encryption technology to automatically encrypt your session without caching.

  • Can I expedite my prescription refill process?

The quickest way to refill a prescription is by contacting your pharmacy directly. Your pharmacist can also address any questions you may have about your medication. Additionally, you can request a prescription refill online by accessing your MyACPNY Portal.

To schedule an appointment, you can directly book with your doctor or use the Find a Provider tool to schedule an appointment at your provider’s office. You can also reschedule with your current provider by logging into the MyACPNY Login portal or calling 646-680-4227 to speak with a Customer Service Representative.

  • Is it possible to access the health records of a family member in MyACPNY?

Yes, it is. By completing a Caregiver Consent Form and submitting it to one of our medical facilities, parents or guardians can log in to their own personal MyACPNY accounts and access family members’ information through the Caregiver Access feature.

  • What should I do if my activation code doesn’t work?

Your activation code expires 30 days after it is issued and is no longer valid after it has been used once, as a security measure. If you are still experiencing difficulties, please contact the MyACPNY Patient Support Line at 646-680-5008.
